We are writing to invite you to join us and many of your colleagues for a six part dialogue on end of life planning from several key aspects. The Closure Initiative/Harrisburg begins on October 13, 2014 and will continue monthly through March 2015. It was developed by the Jewish Healthcare Foundation in Pittsburgh, was launched in Carlisle last winter with great success and is sponsored in Harrisburg by the groups signing this invitation.

It is designed as a “community conversation” that brings a select group of community leaders, clergy, healthcare professionals, policy makers, funders and caregivers together for a series of six discussions related to end-of-life care. Our goal is to learn about this often avoided topic in the mid state, develop an action plan for enhancing awareness and improving it and identify any major policy impediments.

All meetings will be held from 1PM to 3PM and will include lunch. Most will take place in the training rooms at Pinnacle Health’s Community General Osteopathic Hospital. The session dates are:

Monday, October 13, 2014 1PM – at Giant Community Room in Camp Hill
Monday, November 17, 2014 1PM – At Community General
Monday, December 15, 2014 1PM at Community General
Monday, January 5, 2015 1PM at Community General
Friday, February 6, 2015 1PM at Community General
Monday March 16, 2015 1PM at Community General

We are asking that participants make an effort to join us for all six sessions because they build upon each other to support collaborative planning and action. Your experience and interest can do much to inform the discussion
